Tier 5 — Sales & CRM

Lead Capture Forms

Lead Capture Forms put an enquiry form on your own website, your van, your Facebook page or your email signature. Every submission is checked for spam, then lands as a lead in All Leads and the Lead Pipeline. The customer's answers, any photos, and where the enquiry came from land with it.

Applies to: all plans. Viewing forms and their submissions needs crm:leads:view; creating and editing them need crm:leads:edit. There is no sidebar entry — the way in is Sales > All Leads > Options.

Prerequisites: Managing Leads

Creating a form

  1. Go to Sales > All Leads, open Options and choose Lead Capture Forms.
  2. Select New form, give it a name and pick what it's for: a general enquiry, a quote request, a service booking or an emergency callout. Each starts with a sensible set of fields.
  3. On the Fields tab, add, reorder and remove questions. Choose which lead field each answer is saved to — name, phone, email, address, description, or "how did you hear about us". Anything not mapped is kept as a form answer on the lead.
  4. Use Only show this field when… to ask a follow-up question only when it's relevant. An "install detail" box, for instance, can appear only when the customer picks Installation.
  5. Select Save. The live preview on the right shows exactly what customers will see.

The editor has seven tabs: Fields, Design, After submit, Notifications, Protection & privacy, Share & embed and Submissions. The badge beside the form name reads Live or Off; a new form starts Live, and Switch off in the options menu takes it down without deleting it.

Design

Set the headline, intro text and button label, choose a brand colour and corner radius, and decide whether your logo (from Business Settings) is shown. Transparent background lets your website's own background show through when the form is embedded.

After submit

  • Thank-you message or redirect to a page on your site (useful for Google Ads conversion tags).
  • Confirmation email to the customer, sent straight away. Keep it factual — a plain acknowledgement doesn't need marketing consent, offers and promotional links do.
  • Confirmation SMS to Australian mobiles, using your SMS provider.

Notifications and routing

Most businesses never reply to a web enquiry within five minutes, and the first to respond usually wins the job. Choose who is told — in-app, by email and by SMS — and which pipeline stage and team member the lead lands with. Emergency forms should page the on-call phone by SMS.

Every submission also fires the Lead Created and Lead Capture Form Submitted automation triggers, so follow-up sequences and escalations live under Automations.

Protection & privacy

Every form carries a hidden honeypot field, timing checks and rate limits. Blocked submissions are kept under the form's Submissions tab, so nothing is dropped silently and spam never creates a lead or a customer.

On this tab you can also turn on a captcha, which is invisible to almost everyone and only challenges suspicious visitors. It needs captcha keys set on the server; where they are missing the tab tells you the toggle has no effect. Only accept submissions from these websites restricts an embed to your own domains, subdomains included — leave it empty to keep the hosted link and QR codes working anywhere.

The Privacy notice shows under the button, as the Privacy Act expects at the point of collection, alongside a link to your privacy policy. Turn on Ask for marketing consent to add an unticked box for campaign opt-in; the answer is recorded on the lead.

Where the server has address lookup available, Google address autocomplete puts a structured suburb, state and postcode on the lead instead of a free-text line.

Sharing and embedding

The Share & embed tab gives you:

  • A hosted link — works on Facebook page buttons, Google Business Profile, Instagram bios, SMS and email signatures.
  • A QR code to print on the van, flyers and business cards.
  • An embed code for your website that sizes itself to its content (WordPress, Squarespace Business, Shopify, custom sites).
  • A popup button — any button on your page can open the form.
  • A plain iframe for builders that block scripts (Wix, GoDaddy, WordPress.com free plans).

Campaign parameters on the page (utm_source, gclid, fbclid), the referrer and the page the form was on are recorded on every lead. The embed also fires a generate_lead event and pushes ab_form_submitted to your site's dataLayer. Track Google Ads and GA4 conversions from your own tag manager off those.

Tip: add a Hidden (from URL) field to the form, then put ?campaign=van on the hosted link behind each QR code. You'll know which van, flyer or ad produced each lead.

Returning customers and attachments

Where the phone number or email matches an existing customer, AxiomBlue links the lead to that customer instead of duplicating it. The lead's notes say so.

Photos and PDFs the customer attaches are stored against the lead. You will find them in the Form submission card on the lead, next to the answers and the campaign details.

Reviewing submissions

The Submissions tab lists every attempt with its status: lead created, spam with the reasons given, held because your plan's lead limit was reached, or rejected. Held and spam-flagged submissions carry a Create lead button, so nothing genuine is lost.
